Applying for Healthy Start is easy – this is how you do it:Healthy food

  1. Get an application leaflet – see below where you can get it from. You need to fill in Part A of the application form carefully in black ink using CAPITAL letters.
  2. Due to changes in the law, you no longer need to get a health professional to sign your Healthy Start application form. This means you can leave Part B blank. However if you are pregnant you need to complete your estimated due date in Part B. Don’t forget that you/the applicant still needs to sign the application in Part A, Section 7.
  3. Make sure all the information on the form is correct and that you have signed and dated it.
  4. Send the form to us at the address below – either write the address on a plain envelope (you don’t need a stamp) or use the freepost envelope at the back of the application leaflet if you have one.

Do I still need a midwife/health visitor to sign my application form?

Since 6 April, a health professional signature is no longer required.

I’ve completed my application (apart from the healthcare professional signature). Can I just send it in now?

Since 6 April, you no longer need a health professional to sign your Healthy Start application form.  This means you can leave Part B blank. Don’t forget that you or the applicant still needs to sign the application in Part A, Section 7.

Why can’t you just issue vouchers automatically to everyone on benefits? Why do I still need to apply?

Not everyone who is on benefits or tax credits is eligible for Healthy Start vouchers and by law Healthy Start vouchers can only be issued after a completed application is received. If you are eligible for Healthy Start vouchers, you will be invited to apply – all eligible families are sent a partly filled in form for them to complete and send back.

 

How to get the application leaflet

You can get an application leaflet by:

  • asking your midwife or health visitor for one
  • calling the Healthy Start helpline on 0345 607 6823 and asking them to send one to you by post
  • filling in the form online and then printing it off to check and sign
  •  emailing a blank form (PDF) to yourself
  • or downloading it directly from this website. If you are emailing a blank application form to yourself or downloading it from this website, you will need to have the free Adobe Acrobat Reader to view and print it.
